Completing the Debit Card Application

To complete the debit card application, the applicant must enter:

  1. Name
  2. Primary Residence Address (Where you live)
    This is the address that will be used to verify your identity. It must match what is in current public records. The name and address combination found in the public record must match your name and this primary address.
  3. Mailing Address (Where you get your mail and credit card bills)
    Enter a mailing address if you want this card and future mailings about this card to come to this address instead of your primary residence address. If you enter a mailing address and are paying by credit card, you will be asked which address to use (primary residence or mailing) to match what is on file with your credit card company for the credit card billing address. You will tell the system which address to use when you get to the screen to input the credit card number for payment for this card.

Privacy Information

This section is for inputting your personal information

  1. Social Security Number
  2. Date of Birth
  3. Mother's Maiden Name

Payment and Initial Deposit

Tell the system how much you want to deposit on the card. You also tell the system how you want to pay for this card in this section (check, money order, cashier's check, or credit/debit card).
